Consent and Notice of Background Check

League Supplemental Instructions

NOTE TO LEAGUES:  If you will be registering your players using the excel spreadsheet you MUST use the sample spreadsheet template in entering your players.

If you created your own spreadsheet you can cut and paste the information onto the template.

If you are registering your coaches/managers and DID NOT check the ACE w/Background box, then you do not to enter the Driver’s License information.

If you DID check the ACE w/Background box then you MUST include the coaches/manager’s Phone Number, Email Address, Driver’s License Number, State and Expiration Date. 

You must also have each coach complete the “CONSENT and NOTICE OF BACKGROUND CHECK” and submit this form along with your invoice to process.
